OUR PLAN

Age Friendly Sonoma County is a grass roots effort including local volunteers of all ages to promote community awareness to "Changing Aging". We have developed a plan that searches for avenues that link older adults to one another and to resources that can help them stay connected to family, friends and community for as long as possible.

Through the creation of local task forces and the alignment of community leaders and partners, we will continue our outreach and weave age-friendly messaging into all efforts to ensure Sonoma County becomes a Livable Community.
